Eligible hospitals and clinics may order and prescribe Dexcom G6 CGM in compliance with the appropriate guidelines established by the academic society. To receive reimbursement, Healthcare providers will provide instructions to patients or patient families on how to use the Dexcom G6 CGM system. Reimbursement includes the cost of all associated items and the education fee.
Provider Eligibility
Once eligible, providers may order Dexcom G6 CGM, prescribe to patients, and submit for reimbursement.
Requirements:
- At least one full-time physician with 5 years of specialized knowledge and experience in treating diabetes
- A site providing Continuous Subcutaneous Insulin Infusion (CSII)
- At least one full-time nurse or pharmacist who has at least 2 years of experience in Continuous Subcutaneous Insulin Infusion therapy for diabetes and has completed appropriate training (e-learning by JDS) on Continuous Glucose Monitoring.
Patient Eligibility
Patients with Type 1 or Type 2 diabetes who perform at least 2 self-blood glucose measurements per day may be eligible for Dexcom G6 CGM.
Patients must receive a prescription from an approved healthcare provider and will be responsible for the medical service and device co-pay.

Order & Prescribe Dexcom
Provider orders Dexcom G6 CGM from Dexcom distributor to prescribe to eligible patients in hospital or clinic.
Submit for Reimbursement
Provider submits prescription including codes and receipt of payment to insurance company for reimbursement.
